Welcome to the staff of Mt. Lebanon Extended Day Program (MLEDP). You will soon be asked to complete your new hire forms at a scheduled time in the MLEDP Office.

Before you come to the office, please gather the information you will need to complete the hiring process.

Identification:
You will need to bring 2 forms of identification - Examples:

  • Photo Driver’s License
  • Birth Certificate
  • Passport
  • Social Security Card
  • Student ID with Photo

Filling Out New Hire Forms:
In order to be prepared to complete the forms at the office, including clearances, please have the following information ready.

  • Name
  • Address
  • Contact Information - Phone and Email
  • Date of Birth
  • Social Security Number
  • Your Personal Emergency Contact Information

Required Clearances:

  • If you have current clearances (done within the last 5 years) PA Criminal Background, PA Child Abuse History and FBI, please bring them to the office.
  • If you need to do Clearances, you will complete the PA Criminal Background and the PA Child Abuse History Clearances online at the office. Your clearances will be printed at the office.
  • To start the process to secure your FBI Clearance, you will complete the online Registration to go for your fingerprints. It is your responsibility to bring your FBI Clearance. received at your home address to the office as soon as it is received. You jeopardize being able to work, if it is not turned in.
  • For Child Abuse History Clearance: Be prepared to list past addresses and names of people with whom you live and have lived since 1975

Payroll Info:

  1. Name of the town in which you live and the School District in which you reside
  2. Deduction number for W-4
  3. Name of your banking institution and direct deposit info (voided check)

Please Bring With You:

  1. Education Credentials (Diploma and/or Transcript)
  2. Two (2) Letters of Reference - Letters of Reference may be sent by e-mail to: exdir@mledp.org

Before Working at a Center, You Must Also Have:

  • Staff Health Assessment and TB Test - before you can be on site with children your health assessment and TB test must be completed and on file in the office.
  • The Staff Health Assessment Form is on this web site in the Staff Information Tab above.

Mandated Reporter Training will be completed at the office.

This Form is used to have someone else (other than the persons designated on your Parental Consent - Emergency Contact Form) pick up your child at our centers.

We need this Form as a written release from you for the purpose of releasing your child. This procedure is for your child's safety.

Counselor-In-Training (CIT) Information for Summer Fun Program
CIT's are 6th to 12th Grade Students
This is a Volunteer Activity for Community Service Hours
We highly recommend you read each panel before you proceed with registering.

Welcome to the Counselor-In-Training (CIT) section of the Summer Fun Program of MLEDP.

Below you will find all the necessary information and forms that must be filled out in order to participate as a CIT in the Summer Fun Program.

CIT's will be scheduled to volunteer for a few hours, several days a week based on availability. MLEDP staff will guide each CIT. As a CIT you will receive Fire Safety Training, Basic First Aid Review and preparation for the development of the age you will work with.

At the end of the summer, documentation of your volunteer hours will be provided to you and to your school for your records, which will help you fulfill you volunteer community service requirement.

The CIT program is overseen by the Asst. Executive Director & Summer Program Director: Dawn Chrestay

  • To build relationships
  • Provide role models for the children
  • Provide opportunities for taking initiative
  • Making peer influences positive
  • Tapping the desire to make commitments
  • Provide safe opportunities for risk taking
  • Provide positive support and encouragement
  • Encourage responsibility
  • Increase participation by older children
  • Have additional trained eyes, ears, and hands in the program operation
  • Show the ability to work with children and staff
  • Display outstanding character
  • Increase the older child's self-confidence, tolerance, ability to set goals, problem solve, and decision making abilities

Must have completed the 6th grade to be eligible to be a CIT

Participate in an interview (for new applicants only)

Participate in a 2 Hour Training Session that will include:

  • The Philosophy of MLEDP
  • Development of Children: Ages Assigned
  • First Aid Discussion
  • Fire Safety

Completion of the following 7 Registration Forms

  1. CIT Application
  2. CIT Responsibility Form
  3. CIT Calendar of Availability
  4. CIT Minor Release
  5. CIT Reference Letter
  6. CIT Parental Consent & Emergency Contact Form
  7. Child Health Assessment Form

NOTE: If you are new to the program or we do not have a valid Child Health Assessment Form on file (valid for one year), you and your doctor must complete this form prior to you participating in the Summer Fun Program as a CIT.

You may also get a copy of a valid Physical Form that is on file with the school and use as a substitute to the Child Health Assessment Form.

This is a requirement of the Pennsylvania Department of Human Services.
