Permanent Schedule Change Form

Permanent schedule changes are defined as schedule changes of one month’s duration or longer. Request for a permanent schedule change must be made in writing two (2) weeks prior to the desired change. Schedule changes will be accommodated whenever possible. However, space and/or staffing limitations may restrict certain schedule additions or changes. Upon confirmation of the schedule change, a contract addendum with the updated Schedule Payment Agreement will be prepared for your signature.
