The Summer Fun Program provides full day care during the summer vacation for children completing Kindergarten through Grade Five.

Center Location: Mt. Lebanon Christian Church
Open: Monday through Friday
Time: 7:00 a.m.-6:30 p.m.
Note to Parents:
You are invited to a brief meeting to receive important information about the MLEDP Summer Fun Program and to have your questions answered. Parents of children with special needs will have the opportunity to meet with Summer Fun staff immediately following this meeting.

Your Summer Fun Registration Package has a total of 6 Forms

All the forms are Editable PDF Forms  that make it easy to fill out.
Just TAB through the document - TYPE in your information - then PRINT.

The following 6 forms should be downloaded to your computer!

All 6 Enrollment Forms are REQUIRED to be TYPED.
*All 6 Forms MUST be submitted to the office
or your child WILL NOT BE ENROLLED!
1 - Registration Schedule Payment Agreement - Required
2 - Parental Consent & Emergency Contact Form - Required
3 - Sun Screen and Swimming Permission Form - Required
4 - Expectations Checklist - Required
5 - Minor Media Release Form - Required
6 - Child Health Assessment Form - Required (your doctor completes this form)
NOTE: If you are new to the program or we do not have a valid Child Health Assessment Form on file (valid for one year), you and your doctor must complete this form prior to your child participating in the Summer Fun Program. This is a requirement of the Pennsylvania Department of Public Welfare.
A copy of your signed Registration Schedule Payment Agreement will be sent back to you. We will fill in your weekly fee based upon your schedule selections.
Non- Residence please call for Summer Fun Rates
(Non-Residence are those outside the school systems we service during the school year)
If you are registering a child with a special need, you must come to the meeting listed above, so our staff may discuss your child's individual needs. Fill out all required forms On-Line and mail them in.
Due to DPW site capacity limitations our enrollment process has always been a first come, first served basis at our sites. So please, send in your enrollment forms as soon as possible.
