You must fill out all forms to apply.
Please Read all information carefully!
All the forms are Editable PDF Forms that make it easy to fill out.
Just TAB through the document - TYPE in your information - then PRINT.
It's just that easy!

For Ease of Printing
The following 7 forms are to be should be downloaded to your computer

You can now SAVE the information you type in the forms for future use. You will need the latest version of Adobe Reader.
You will need Adobe Reader to open these files.

All 7 Enrollment Forms are REQUIRED to be TYPED.
1 - CIT Application - Required
2 - CIT Responsibility Form - Required
3 - CIT Calendar of Availability - Required
4 - CIT Minor Release - Required
5 - CIT Reference Letter - Required
Note: You will need to print 2 copies of this form. You need 2 references to apply.
6 - CIT Parental Consent & Emergency Contact Form - Required
7 - Child Health Assessment Form - Required
NOTE: If you are new to the program or we do not have a valid Child Health Assessment Form on file (valid for one year), you and your doctor must complete this form prior to your child participating in the Summer Fun Program as a CIT. You may also get a copy of a valid Physical Form that is on file with the school and use as a substitute to the Child Health Assessment Form. This is a requirement of the Pennsylvania Department of Public Welfare.
