School Year Care Program

School Year Care Program Information - Parent Handbook
We highly recommend you read each panel before you proceed with registering.

NOTE: The School Year Care Registration Form for the 2018- 2019 school year will be accepted on Monday, July 9, 2018 starting at 8 AM.

Note: There is a required weekly minimum enrollment:
Morning Care has a minimum of 2 scheduled days per week
Afternoon Care has a minimum of 3 scheduled days per week

The School Year Care Program is a complement to the school day, giving children a safe place to relax, be with friends, do homework, play games, and most importantly have fun.

The School Year Care Program provides before and after school care for children in grades 1 through 5.

Our centers are located within the following schools: Foster, Hoover, Howe, Jefferson, Lincoln, Markham and Washington Elementary School.

This is not a continuation of the school day.

 

Morning Session - 7:00 to 8:30 a.m.
Afternoon Session - 3:30 to 6:30 p.m.
Monday through Friday.

Invoices Reflect your Submitted Schedule (RSPA) of Typical School Year Calendar Days. These days are totaled and then divided into Easy Monthly Installments. Payment is due on the first of each month. Long Days and Half Days are billed separately and are not part of your RSPA agreement.

There is a non-refundable registration fee per child.
MLEDP reserves the right to change rates at any time.

We do not bill for ACT 80 days and Early Dismissal.

*A Typical School Day is a day when children are scheduled for a regular session of school.

*Typical Days Billing: The Typical Days listed above in the School Year Care Schedule Calendar, are totaled (based on you RSPA agreement) then divided into easy monthly installments.

 

The following items are billed separately.

  1. **Long Days are in service-clerical days and parent teacher conference days with 11.5 hours of care available.
  2. **Long Days are always billed separately. Since families sign up for care as needed and Long Days are an additional cost, we do not pre-bill for these days. You are only billed for the days for which you sign up. No Tuition Credits are Given for absences on Long Days.
  3. Late Pick Up Fee - $10.00 for every 15-minute increment beyond closing time (6:30 p.m.).
  4. Late Payment Fee - Payments are due the first day of every month. A late fee of 5% of the account balance will be assessed to accounts, if payment arrives after the 5th day of the month.
  5. Failure To Call Off Fee: If your child is scheduled for afternoon care and is absent without calling to inform the center, then you will be charged a $25.00 Failure To Call Off Fee.

There are two (2) School Year Care Enrollment Forms:
1 - School Year Care Registration Form
2 - Child Health Report Form

NOTE 1: If you are new to the program or we do not have a valid Child Health Assessment Form on file (valid for one year), you and your doctor must complete the Child Health Report Form prior to your child participating in the School Year Care Program.

This is a requirement of the Pennsylvania Department of Human Services

NOTE 2 - AND THIS IS A BIGGIE

In Adobe Acrobat Reader, once you use the Fill & Sign Tool > Sign, and place a signature on the document, and then SAVE the document, you CAN NOT ALTER the form or add anything. Saving the form with a signature on it, locks the form. This is a security function of Adobe Acrobat Reader.

Saving the document with an electronic signature is the key function. You can add signatures as you move through the document, as long as you DO NOT SAVE it until it is completely filled out.

Once you Save a form with an Electronic Signature on it
(using the Fill & Sign Tool > Sign) it is LOCKED FOREVER

Suggestions - Recommendations:

  1. Fill out the form in one sitting, and DO NOT save the form until you have completed the entire form with all appropriate information and all signatures
  2. If you plan to complete the form over time or just want to be safe, we suggest that you DO NOT add any signatures until the form is completely filled out, then add all your signatures at one time at the end of the process. Use the checklists for each section as a guide to make sure you have all the appropriate signatures for each section

  • The two (2) Enrollment Forms are REQUIRED TO BE TYPED
    (DO NOT SUBMIT HANDWRITTEN FORMS - You wil be asked to RE-DO your forms)
  • You must fill out every page, including the Checklists and return all pages to MLEDP

DELAYED START - On the mornings that there is a Delayed Start for school, the school-age and kindergarten care centers will open at their normal time; 7:00 a.m.

Special Note: On days when there is a School Delay and if there are Unsafe Weather Related Travel Conditions for our staff, MLEDP reserves the right to open later or close for the morning session. Unsafe Weather Related Travel Conditions usually means there is: black ice, icy snow-covered roads, or impassable roads for our staff. FYI: When this unusual situation occurs, we provide these two media outlets with our Delay or Closing information: KDKA-TV and WPXI-TV. They usually post it on their websites as well.

A DELAY BECOMES A CLOSING - If a Delay becomes a School Closing while your child is in our care, parents will be notified of the closure by phone and will need to pick up their child.

SCHOOL CLOSED PRIOR TO 7AM - If the school district announces that the schools are closed prior to 7:00 a.m., MLEDP is also closed.

Tuition Credits Not Given
Credits are not given for absences, illnesses, vacations, announced school closings, or closings due to weather, water, or emergency situations.

On scheduled district-wide in-service days, when school classes are not in session, but the school building is open, MLEDP will operate for the whole day; 7 a.m. - 6:30 p.m.

Families have the opportunity to sign up for these days, if they need child care.

The fee charged for in-service days is not included in the contracted care and will be billed separately. Due to traditionally low attendance on these days, MLEDP combines sites.

Parents will receive information about programming and center locations for these days from their center director.

Mt. Lebanon Extended Day Program is required by the DHS to have parents review their important information, at a six-month interval after enrollment.

After reviewing your child’s information from your current Registration Schedule Payment Agreement and your Parental Consent & Emergency Contact Form, download the Periodic Review Form, complete it and return it to MLEDP either by Email or regular mail.

Permanent schedule changes are defined as schedule changes of one month's duration or longer. Request for a permanent schedule change must be made in writing two (2) weeks prior to the desired change. Schedule changes will be accommodated whenever possible. However, space and/or staffing limitations may restrict certain schedule additions or changes.

Upon confirmation of the schedule change, a contract addendum with the updated Schedule Payment Agreement will be prepared for your signature.
